Fees and Billing

My fee for a 45-50 minute individual therapy session is $140; for couples, my fee is $160. I also offer a limited number of sliding scale arrangements for those who have a demonstrated need. My fee for group work is $60 for a 90 minute session. These fees are due at each session (or for group work, at the end of the month).

As a Licensed Psychologist, I am insurance eligible for all insurance carriers in Washington State. I am a preferred, in-network provider for Premera Blue Cross, Regence Blue Shield, LifeWise, Uniform Medical Plan, and First Choice. For in-network services, I will bill your insurer on your behalf. To assist in obtaining reimbursement for out-of-network services, I will provide you with an invoice to submit to your insurer. Unless I am an in-network provider, payment in full is due at the time of serivce.

The decision whether to use managed care for therapy can be complex and varies from person to person. Insurers have the right to limit the number of sessions for which they will pay, and often they will require the periodic release of information that will justify, in their eyes, the cost of therapy. I am happy to speak with you about the decision whether to include your benefit company in your treatment.

If you plan to use your insurance for therapy, you are strongly advised to call your insurer prior to our first meeting to be clear of your benefits and how they pertain to our work.

Cancellation/Late Attendance Policies

You may cancel an appointment with at least 24 hours notice. If you have not given 24 hours notice, you'll be responsible for paying for your reserved time slot. Of course, benefit companies will not cover missed appointments. This payment will be due at our next meeting.

Please note that in the case of a late arrival, appointments will still end at their scheduled time.

Documents and Forms for New Clients

So that our time together can be spent most productively, please print and complete the following 3 forms prior to our first meeting:

1. Disclosure and Consent to Treatment.pdf. The Disclosure and Consent form provides information on various aspects of our therapeutic agreement. Please familiarize yourself with this information. You will need to sign this form (on pa 7) before we may begin our work. Of course I'm happy to discuss and clarify anything you'd like prior to your signing.

2. Privacy Notice.pdf. The Privacy Notice provides more detailed information on your rights to confidentiality. This form is yours to keep and no signature is required.

3. Intake Form for New Clients.pdf. The Intake Form typically takes 15 minutes or less to complete and allows me to more quickly understand what may be bringing you to therapy.

Note: If you will be relying on your insurance company to pay for all or part of your treatment, please print and complete the Benefit Company Authorization Form prior to our meeting.
